What is a medical card?
If you are at least 18 with one of the qualifying medical conditions, then you are eligible for a medical card. A medical card will allow you to purchase medical cannabis products and pay lower taxes on them at 6%.
We’ve partnered with First Natural Wellness, which offers in-person and virtual appointments. First Natural Wellness also offers patients an Instant Medical Card at no extra cost.

4 Steps Easy Steps
Step 1: Complete a physician certification form
Visit a physician with which you have a bona fide doctor/patient relationship. Receive certification that you suffer from one of the condition that qualifies you to receive a patient registry identification card.
The MMMP reserves medical cannabis access for patients with the following debilitating medical conditions:
- Alzheimer’s disease
- Amyotrophic lateral sclerosis (ALS), or Lou Gehrig’s disease
- Cancer
- Glaucoma
- Hepatitis C
- HIV/AIDS
- Crohn’s disease
- Nail–patella syndrome
- PTSD
The MMMP also reserves medical cannabis for chronic or debilitating conditions, or their treatments, that produce one or more of the following:
- Cachexia, or wasting syndrome
- Seizures, including but not limited to those characteristic of epilepsy
- Severe and chronic pain
- Severe nausea
- Severe and persistent muscle spasms, including but not limited to those characteristic of multiple sclerosis
Step 2: Complete an application form
Include proof of Michigan residency (one of the following):
- A copy of your valid, lawfully obtained Michigan driver license
- A copy of your valid, lawfully obtained Michigan personal identification card
- A copy of your valid Michigan voter registration (If you submit a voter registration as proof of Michigan residency, you must submit proof of your identity. Proof of identity can be any government–issued document that includes your name and date of birth)
If you are designating a caregiver, include proof of the caregiver’s identity (one of the following):
- A copy of the caregiver’s valid state–issued driver license
- A copy of the caregiver’s valid state–issued personal identification card
Step 3: Submit the required application fee
Include a check or money order made payable to: State of Michigan–MMMP
- Patient fee: $40
- Caregiver fee: FREE
Step 4: Mail in one envelope
